- Focus on providing palliative care to patients and their families
- Assist families along their healthcare journeys with love and compassion
- Collaborate with the healthcare team to develop and implement patient care plans
- Participate in ongoing education and professional development opportunities to enhance skills and knowledge
- Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing
- 2 years of experience within the last 5 years in chronic disease management/long-term care. Hospice background preferred
