Key Responsibilities:


The Case Manager evaluates the social work requirements of patients and implements social work strategies as part of the discharge planning process.


Core Functions in Case Management Include:
- Clinical Interventions and Management of Discharge and Care Planning
- Financial Oversight
- Management of Payer and Referral Processes

Specific Duties of Case Managers:
- Coordination of healthcare decisions through a systematic approach to ensure treatment plans that enhance quality and outcomes
- Management of care across the healthcare continuum
- Promotion of cost-effective care within designated timeframes
- Assurance of payment for hospital services that meet patient-related utilization management criteria
- Development and execution of safe and appropriate discharge plans

Qualifications:
Essential Requirements - Social Work Background:
Current and valid state licensure in Social Work preferred - LCSW or LSW

Essential Requirements - RN Background:
Current state licensure as a Registered Nurse required

Preferred Qualifications:
- Bachelor of Science in Nursing (BSN)
- Certification in Case Management (CCM)
- Prior experience in case management and discharge planning

Per Diem Expectations:
Two shifts within a 4-week schedule as census supports, including two high-need staffing shifts, minor and major vacation periods.
