Position Summary:
As a vital member of the healthcare team, the Patient Care Coordinator serves as the primary liaison for referring healthcare providers, patients, families, and caregivers. This role is essential in facilitating access to clinical resources, supportive care, and financial assistance available within the St. Joseph's Health system and the surrounding community.


Key Responsibilities:
1. Act as the main point of contact for all stakeholders, ensuring seamless communication and support.
2. Collaborate with a multidisciplinary team to coordinate services and maintain continuity of care for patients.
3. Proactively identify potential barriers to care and implement strategies to ensure timely and efficient service delivery.
4. Oversee transitions of care, ensuring patients receive appropriate referrals and follow-up appointments.
5. Provide comprehensive patient education to promote adherence to care plans and enhance health outcomes.

Required Qualifications:
- A Bachelor's degree in Nursing or a closely related discipline is preferred.
- A minimum of three to five years of relevant professional experience is required.
- Must possess a valid Registered Nurse license issued by the State of New Jersey.
- Completion of department-specific orientation and Basic Life Support (BLS) certification within the first year of employment is mandatory.
- Proficiency in Microsoft Excel and Word, along with strong data entry skills, is essential for this role.
