Job Title: LPN Nurse

BrightSpring Health Services is seeking a skilled LPN Nurse to join our team. As a Graduate Nurse, you will be responsible for providing high-quality patient care and working closely with our team of healthcare professionals.

Responsibilities:
  • Provide comprehensive medication reviews and ensure patients and their caregivers receive written information regarding their medication regimen.
  • Develop, implement, and update individualized plans of care for patients in partnership with the patient, caregivers, and physician.
  • Participate in critical thinking skills development and provide services and treatments to patients as ordered by the physician or allowed practitioner.
  • Monitor and evaluate patient response to treatment, medication, and healing, and communicate with the patient's physician or allowed practitioner regarding patient needs and changes in condition.
  • Provide patient and caregiver education on diagnosis, medications, infection control, and safety measures in accordance with the plan of care.
  • Prepare clinical notes by documenting observations, treatment provided, and patient response to treatment, and submit documentation to the agency in a timely manner.
  • Participate in the agency's QAPI program by communicating problem areas identified and recommendations to address problem areas, data collection, attendance at periodic QAPI meetings, and participates in performance improvement projects as requested.
  • Maintain universal precautions and infection control standards to prevent the transmission of infections and communicable diseases.
Qualifications:
  • Recent graduate of an approved school of professional nursing within the last six months.
  • Successfully passed NCLEX Exam within the first ninety days of hire.
  • Current CPR certification.
  • Basic clinical skills.
  • Professional appearance and demeanor.
  • General knowledge of computers and the ability to learn industry-specific software applications.
  • Excellent oral, written, and interpersonal communication skills.
  • Excellent organizational and time management skills with a keen attention to details.
  • Ability to work effectively in a team environment.
