Responsibilities:
Role of a Healthcare Relationship Coordinator

As an advocate for patients during the pre-admission phase, you will:

Work under the guidance of the Director of Business Development, collaborating with clinical teams to assess potential patients' conditions and medical requirements.

Develop strong relationships with healthcare professionals and referring medical facilities.


Our hospitals implement a post-ICU care plan emphasizing mobility, nutrition, and rehabilitation - all vital for helping patients regain their health.

Assist local hospitals in identifying post-ICU patients who are experiencing chronic critical illness and would benefit from our specialized extended acute care services.


Collaborate with physicians, discharge planners, and families to ensure the best continuum of care following an ICU or surgical stay, utilizing your clinical expertise, payer knowledge, and problem-solving abilities.


Conduct clinical evaluations of referred patients and guide families in making informed care decisions for their loved ones.


Partner with hospital case managers and physicians to streamline pre-admission and transfer processes by addressing payer-related and operational challenges for patients accessing our effective care level.


Qualifications:

Minimum requirements:
Registered Nurse (RN) or Respiratory Therapist (RT) License.

Other Clinical Licenses may be accepted with prior approval from HR and hospital leadership.

Preferred qualifications for success:


Be an investigator and problem solver – leverage your critical thinking skills to navigate obstacles preventing optimal patient admissions.

Demonstrate flexibility, self-motivation, accountability for outcomes, and support for team initiatives.

Prior experience in sales or business development is advantageous, though comprehensive training will be provided.
