Role Responsibilities:
Prepare patients for their appointments by executing the Advanced Standard room process, which includes documenting the chief complaint, measuring vital signs, reviewing medical history and allergies, conducting thorough medication reconciliation, and utilizing the Encounter Guide to address any outstanding care gaps such as fall risk assessments, depression screenings, and necessary preventative health checks. This also involves rechecking elevated blood pressures and completing essential documentation in Epic for annual preventative visits as required.


Assist with various in-office procedures, including but not limited to, blood draws, mental status evaluations, EKGs, walking oxygen studies, and point-of-care testing.

Administer vaccinations and other injections as permitted within the scope of practice.

Schedule follow-up appointments as per office protocols.

Collaborate with healthcare providers and the clinical care team to oversee and coordinate the implementation of care plans.



LPN Qualifications:

Education:
Diploma in Practical Nursing

Licensure:
Active LPN licensure

Preferred Experience: 6 months of clinical healthcare experience, ideally in a primary care setting.

MA Qualifications:

Medical Assistant:

Education:
High School Diploma or GED.

Certification:
Must obtain BLS certification within 90 days of hire.
