Weekend Intake Coordinator for Home Health Services

Job Summary: The Intake Coordinator will be responsible for overseeing the initial intake process for patients referred to our home health services. This will involve collaborating with patients and healthcare providers, gathering essential information, coordinating assessments, and ensuring a seamless transition into home-based care.

Key Responsibilities:
  • Receive and process new patient referrals using our industry-leading EMR system
  • Perform accurate and timely data entry
  • Assign new patients to a Case Manager to ensure the admissions visit is completed within 48 hours of referral receipt
  • Communicate with various healthcare professionals to provide the highest level of care
  • Use HIPAA-compliant eFax system to receive and process orders for new and current patients
  • Perform follow-up calls to patients and families to offer clear and supportive communication
  • Perform other various coordination duties to support day-to-day workflow


Work Environment: We are hiring for the following locations: Burlingame, San Jose, Oakland, and Fairfield. We offer a variety of schedule options, including any 5-day schedule that includes Saturday and Sunday, as well as weekdays in-office and weekends remote.

Compensation and Benefits:
  • $40.00-$50.00 per hour
  • This position is also eligible for monthly productivity bonuses
  • Compensation is determined by factors such as experience, knowledge, skills, location, internal equity, and market data

Requirements:
  • Two years of administrative experience, preferably in a medical setting, required
  • Direct Intake and/or patient registrations/admissions experience highly desired
