Job Summary:
We are seeking a skilled Occupational Therapist to join our team at HexaQuEST Health, Inc. as a Home Health Occupational Therapist. As a key member of our rehabilitation team, you will be responsible for providing high-quality occupational therapy services to patients in their homes.

Responsibilities:

  • Prioritize start of care visits to ensure timely and effective care delivery
  • Supervise Occupational Therapy Assistants to ensure quality care and adherence to company standards
  • Maintain accurate and timely clinical notes and documentation
  • Communicate effectively with supervisors, intake, and office staff to ensure seamless care coordination
  • Respond promptly to phone calls and emails during normal business hours

Requirements:

To be successful in this role, you must have a current CA OT License and at least one year of experience in Home Health.

Work Environment:

As a Home Health Occupational Therapist, you will work in a fast-paced environment, visiting patients in their homes and providing individualized care. You will be required to travel to various locations, including Alameda, CA, and potentially expand to other areas as needed.

Benefits:

We offer a competitive bill rate, mileage reimbursement, and parking and toll reimbursement. You will also have the option to use a company car with a decreased bill rate.

Productivity Requirements:

To meet our productivity requirements, you must achieve 30 points per week, which is equivalent to 40 hours of work per week.

Position Urgency:

This is a normal urgency position, and we are looking for a candidate who can start as soon as possible.

Shifts:

This position requires working days, Monday-Friday, with one weekend rotation per month.

State License Details:

You must have a current and active CA OT License.

Minimum Years of Experience:

We require at least one year of experience in Home Health.

Minimum Guaranteed Hours:

You will be guaranteed 40 hours of work per week.

Specialty Type:

This is a Rehabilitation Therapy position, specifically focusing on Occupational Therapy.

Sub Specialty:

You will be working as an Occupational Therapist.

Bilingual:

This position does not require bilingual skills.

EMR Used:

We use the Kinnser EMR system.

Holiday Coverage Required:

You will not be required to work holidays.
