Care Manager

The RN Care Manager is a key member of our interdisciplinary team, responsible for coordinating patient care and services. This role requires a collaborative approach, working with patients, families, care teams, payers, and external entities to ensure a safe transition from the facility to the next provider or care setting.

- Develop and implement individualized care plans, taking into account patient needs, preferences, and goals.
- Collaborate with healthcare providers, payers, and community resources to ensure seamless care transitions.
- Provide education and training to patients, families, and care teams on care management, disease management, and health promotion.
- Conduct thorough assessments of patient needs, identifying opportunities for improvement and developing strategies to address them.
- Develop and maintain relationships with external partners, including payers, community resources, and healthcare providers.
- Stay up-to-date on industry trends, best practices, and regulatory requirements, applying this knowledge to improve patient care and outcomes.
- Nursing diploma (associate's or bachelor's degree in nursing)
- Currently licensed/registered in applicable State. Must maintain an active Registered Nurse (RN) license in good standing throughout employment.
- One (1) year of clinical experience in post acute care setting preferred
- Prior case management, utilization review, and discharge planning experience preferred
- Certified Case Manager (CCM) or Board Certification in Nursing Case Management (RN BC) preferred
This role requires the ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities. The RN Care Manager must be able to communicate effectively with patients, families, and care teams, as well as work collaboratively with external partners.
