RN Care Manager Hybrid

We are seeking a skilled RN Care Manager to join our team at UnitedHealth Group. As a key member of our care management team, you will play a critical role in ensuring the continuity of care for our patients.
Key Responsibilities- Prioritize patient care needs and address emerging issues in a timely and effective manner.
- Collaborate with patients, families, and caregivers to develop and implement personalized care plans.
- Identify and assist high-risk patients in acute care settings, skilled nursing facilities, and ambulatory settings.
- Consult with physicians and other team members to ensure successful implementation of care plans.
- Coordinate treatment plans with the care team and triage interventions appropriate to the skill set of team members.
- Monitor and coach patients using motivational interviewing and behavioral change techniques to maximize self-management.
- Oversee provisions for discharge from facilities, including follow-up appointments, home health, social services, and transportation.
- Work in coordination with the care team and demonstrate accountability for patient management and outcomes.
- Maintain effective communication with physicians, hospitalists, extended care facilities, patients, and families.
- Provide accurate information to patients and families regarding resources available to them through health plan benefits, community resources, and referrals.
- Participate actively in monthly care management department meetings and daily huddles.
- Document pertinent patient information and care management plans in the electronic health record.
- Coordinate care with central departments on assigned patient caseload, including inpatient, long-term care facilities, adult family homes, and home health agencies.
- Demonstrate a thorough understanding of the cost consequences resulting from care management decisions through utilization reports and systems.
- Graduation from an accredited school of nursing.
- Washington State Registered Nurse license.
- Basic Life Support for Healthcare Providers (AHA) or CPR/AED for the Professional Rescuer (American Red Cross).
- 3+ years of experience in a clinical setting.
- Washington State driver's license and vehicle for work-related travel.
- Bachelor of Science in Nursing, BSN.
- Telehealth certification.
- 3+ years of experience working in acute care.
- 1+ years of care management, utilization review, or discharge planning experience.
- HMO experience.
