Position Overview

The RN Care Manager plays a pivotal role in orchestrating the comprehensive care and services for patients. This position emphasizes the delivery of high-quality care through a collaborative approach that involves coordinating, monitoring, and assessing services tailored to the unique needs of each patient. The RN Care Manager collaborates closely with patients, their families, the care team, insurance providers, and external organizations to ensure a seamless transition from one care setting to another, all while adhering to relevant laws, regulations, and the standards set forth by Marysville Care Center.

Qualifications and Experience
  • Possession of a nursing diploma (either an associate's or bachelor's degree in nursing).
  • Current licensure as a Registered Nurse (RN) in the applicable state, with an obligation to maintain an active license in good standing throughout employment.
  • A minimum of one (1) year of clinical experience in a post-acute care environment is preferred.
  • Experience in case management, utilization review, and discharge planning is advantageous.
  • Certification as a Case Manager (CCM) or Board Certification in Nursing Case Management (RN BC) is preferred.

  • Contribute to the achievement of organizational goals and support strategic initiatives.
  • Possess expert knowledge in the field of nursing practice.
  • Make independent decisions when circumstances necessitate such actions.
  • Maintain familiarity with the practices, procedures, laws, regulations, and guidelines governing operations in a post-acute care facility.
  • Implement and interpret departmental programs, goals, objectives, policies, and procedures.
  • Demonstrate proficiency in all competency areas, including patient rights, safety, and sanitation.
  • Foster professional relationships with colleagues, vendors, and other stakeholders.
  • Uphold the confidentiality of all proprietary and sensitive information.
  • Adhere to company policies, including those related to harassment and compliance.
  • Exhibit integrity and professionalism by following the Code of Conduct and completing mandatory compliance training.
Essential Duties
  • Utilize relevant computer systems efficiently, including data entry and retrieval.
  • Act as a liaison between external case managers, families, physicians, and community resources.
  • Provide training and education to patients, families, associates, and other care providers.
  • Implement standards of practice for care management, ethical performance, and coordination of care functions.
  • Deliver excellent customer service and maintain a positive demeanor towards patients.
  • Assist in the safe evacuation of patients when necessary.
  • Demonstrate reliable and consistent attendance.
  • Employ reasoning skills and sound judgment in all situations.
  • Communicate effectively and work collaboratively within an interdisciplinary team.
  • Engage in physical activities as required, including sitting, standing, bending, lifting, pushing, pulling, stooping, walking, reaching, and moving intermittently during working hours.
  • Read, write, speak, and comprehend the English language proficiently.
