The RN Care Manager plays a pivotal role in orchestrating the care and services provided to patients. This position emphasizes the delivery of high-quality care through a collaborative approach that coordinates, monitors, and assesses services tailored to the unique needs of each patient. The RN Care Manager works closely with patients, their families, the care team, payers, and external organizations to ensure a seamless transition from one care setting to another, adhering to all relevant laws, regulations, and standards set forth by Life Care.
Qualifications and Experience
- Possession of a nursing diploma (either an associate's or bachelor's degree in nursing).
- Active and valid nursing license in the applicable state, with a commitment to maintaining good standing throughout employment.
- A minimum of one year of clinical experience in a post-acute care environment is preferred.
- Experience in case management, utilization review, and discharge planning is advantageous.
- Certification as a Case Manager (CCM) or Board Certification in Nursing Case Management (RN BC) is preferred.
- Produce written communications that are clear, concise, and well-structured.
- Exhibit strong organizational skills, efficiently prioritizing and managing time and tasks.
- Contribute to the organization's goals and support strategic initiatives.
- Demonstrate expert knowledge in the field of nursing practice.
- Make independent decisions when necessary, based on sound judgment.
- Stay informed about practices, procedures, and regulations governing post-acute care facilities.
- Implement and interpret departmental programs, goals, policies, and procedures.
- Perform competently in all areas, including patient rights, safety, and sanitation.
- Maintain professional relationships with colleagues, vendors, and other stakeholders.
- Safeguard the confidentiality of all proprietary and sensitive information.
- Adhere to company policies, including those related to harassment and compliance.
- Uphold integrity and professionalism by following Life Care's Code of Conduct and completing required training.
- Utilize relevant computer systems effectively, including data entry and retrieval.
- Act as a liaison between external case managers, families, physicians, and community resources.
- Provide training and education to patients, families, staff, and other care providers.
- Implement standards of practice for care management, ensuring ethical performance and coordination of care.
- Exhibit excellent customer service and maintain a positive demeanor towards patients.
- Assist in the safe evacuation of patients when necessary.
- Demonstrate consistent attendance and reliability.
- Employ reasoning skills and sound judgment in decision-making.
- Communicate effectively and collaborate productively within an interdisciplinary team.
- Engage in physical activities such as sitting, standing, bending, lifting, and walking as required during working hours.
- Possess the ability to read, write, speak, and understand the English language.
