Nurse Care Coordinator

We are seeking a skilled RN Care Manager to join our team at Optum. As a RN Care Manager, you will play a critical role in ensuring the continuity of care for our patients. This is an exciting opportunity to work in a dynamic and fast-paced environment, utilizing your advanced nursing knowledge and skills to make a meaningful impact on patient outcomes.
Key Responsibilities- Patient Care Coordination: Prioritize patient care needs upon initial visit and address emerging issues.
- Collaboration with Care Team: Meet with patients, patients' family and caregivers as needed to discuss care and treatment plan.
- High-Risk Patient Identification: Identify and assist with the follow-up of high-risk patients in acute care settings, skilled nursing facilities, custodial and ambulatory settings.
- Care Plan Implementation: Consult with physician and other team members to ensure that care plan is successfully implemented.
- Treatment Plan Coordination: Coordinate treatment plans with the care team and triage interventions appropriate to the skill set of the team members.
- Patient Education and Support: Monitor and coach patients using techniques of motivational interviewing and behavioral change to maximize self-management.
- Discharge Planning: Oversees provisions for discharge from facilities including follow-up appointments, home health, social services, transportation, etc. in order to maintain continuity of care.
- Accountability and Communication: Works in coordination with the care team and demonstrates accountability with patient management and outcome. Maintains effective communication with the physicians, hospitalists, extended care facilities, patients, and families.
- Resource Utilization: Provides accurate information to patients and families regarding resources available to them through health plan benefits, community resources, and referrals.
- Documentation and Reporting: Documents pertinent patient information and Care Management Plan in Electronic Health Record. Coordinates care with central departments on assigned patient caseload, including, inpatient, long term care facilities, adult family homes, and home health agencies.
- Cost-Conscious Decision Making: Demonstrates a thorough understanding of the cost consequences resulting from Care Management decisions through utilization reports and systems such as Health Plan Benefits, CM dashboards and reports.
- Education: Graduation from an accredited school of nursing.
- Licensure: Washington State Registered Nurse license.
- Certifications: Basic Life Support for Healthcare providers (AHA) or CPR/AED for the Professional Rescuer (American Red Cross).
- Experience: 3+ years of experience in a clinical setting.
- Transportation: Washington State driver's license and vehicle for work-related travel.
- Education: Bachelor of Science in Nursing, BSN.
- Certifications: Telehealth certification.
- Experience: 3+ years of experience working in acute care.
- Experience: 1+ years of care management, utilization review or discharge planning experience.
- Experience: HMO experience.
