Youth Behavioral Health Specialist

Community Youth Services is seeking a dedicated Transitional Age Youth (TAY) Therapist to provide essential behavioral health services to young people aged 15 to 24.
The ideal candidate will have a Master's Degree in Social Work, Psychology, Counseling, or a related field, and two years of experience in providing clinical services in homes, communities, and schools.
Key Responsibilities:
- Conduct clinical assessments, documentation, case notes, and plans for treatment, service, and discharge.
- Collaborate with systems like Juvenile Justice, mental health services, community supports, schools, and DCYF.
- Provide therapy and support services in homes, schools, and communities, adhering to MAP protocols.
- Manage a caseload of 15-25 clients, depending on case complexity.
- Document data as mandated by MAP and other regulatory bodies.
- Engage in regular meetings, supervision, and consultations.
- Ensure ongoing communication and care coordination with an integrated service team.
- Track client outcomes using MAP tools and adapt treatment interventions accordingly.
Requirements:
- Mental Health Professional credential.
- Child Mental Health Specialist credential.
- Licensed as a Mental Health Counselor, Marriage and Family Therapist, Psychologist, Social Worker, Mental Counselor Associate, Marriage and Family Therapist Associate, or Social Worker Associate.
