Clinical Care Coordinator Role

Catholic Community Services, Family Behavioral Health Services LLC, is seeking a Clinical Care Coordinator to provide innovative mental health services to children, youth, and families.

As a Clinical Care Coordinator, you will work with our enthusiastic clinical teams to provide individualized, creative, and flexible services infused with evidence-based and evidence-informed strategies.

Key Responsibilities:

  • Assess for immediate safety and stabilization needs
  • Develop treatment plans and safety and crisis plans
  • Facilitate child and family team meetings
  • Provide ongoing assessment and evaluation of treatment planning strategies, goals, and outcomes

Requirements:

  • Extensive training in multiple clinical approaches
  • Ability to customize helping approaches to fit the family's uniqueness
  • Consult with clinical supervisor and mental health specialists as needed
