Qualifications:

  • Bachelor's in Social Work, Psychology, Counseling, or a related field, or an Associate's with two years of experience in care coordination or case management. Must have Agency Affiliated Counselor credentials in Washington State.
  • Familiarity with Mental Health, Juvenile Justice, Schools, DCYF, DSHS, DVR, and community support systems.
  • Understanding of cultural issues affecting marginalized youth, commitment to Positive Youth Development and Wraparound principles, and a non-judgmental approach.
  • Knowledge of legal processes concerning children and families, including dependency orders and juvenile court procedures.
  • High level of initiative, efficiency, organizational skills, and the ability to meet deadlines.
  • Proficient in leading meetings, public speaking, task coordination, and building diverse relationships.
  • Skilled in Microsoft Office, particularly MS Word and Outlook, and adept at using the internet and intranet.

Key Responsibilities:

  • Provide comprehensive Care Coordination in home, school, and community settings, adhering to Wraparound principles.
  • Manage a caseload of 10-15 youth, focusing on their needs and strengths.
  • Organize and lead Wraparound Team Meetings, ensuring inclusive and outcome-driven agendas.
  • Foster a supportive team culture, offering a safe space for youth and families.
  • Maintain a positive and accepting approach towards youth and family choices.
  • Exhibit leadership and conflict resolution skills to integrate diverse viewpoints.
  • Keep accurate records as required by "Managing and Adapting Practices" and associated networks.
  • Participate in necessary meetings and consultations, offering flexible availability.
  • Enhance natural support networks for youth and families within the community.
  • Ensure consistent communication and collaboration with Wraparound teams and partners.
  • Provide tailored resources and information to families for a clear understanding of the WISe process.
  • Apply consensus-building and conflict management to develop unified care plans.
  • Document all processes and progress in line with prescribed guidelines.

About Agency Qualifications:

  • Succesfully clear a thorough background check with no history of causing harm, particularly to children or vulnerable adults.
  • Maintain a clean criminal record, demonstrating professional and personal integrity.
  • Hold a valid Washington State Driver's License with at least five years of driving experience and a good driving record for the past three years.
  • Provide proof of auto insurance with coverage of $100,000 or more.
  • Show cultural sensitivity and the ability to respond to the diverse needs of the organization's community.
  • Acquire CPR/1st Aid/Bloodborne Pathogen certification within 30 days of employment (required before starting some positions).
  • Possess strong interpersonal skills and the ability to collaborate effectively within a team.
