Licensed Practical Nurse Home Health Per Diem

CenterWell Home Health is seeking a skilled Licensed Practical Nurse (LPN) to join our team on a per diem basis. As a Home Health LPN, you will provide high-quality patient care in the comfort of their own homes.
Job SummaryWe are looking for a compassionate and detail-oriented LPN to provide skilled nursing interventions, teach patients and their families, and work collaboratively with our interdisciplinary team to achieve optimal patient outcomes.
Responsibilities- Provide professional nursing services under the direction of a RN in compliance with state and federal regulations, as well as CenterWell Home Health policies and procedures.
- Develop and implement individualized plans of care to meet the unique needs of each patient.
- Confer with RN Supervisors and other healthcare professionals to ensure seamless care coordination and communication.
- Document patient progress and outcomes in a timely and accurate manner.
- Participate in ongoing education and training to stay current with best practices and industry developments.
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Valid drivers license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.
