Licensed Practical Nurse Home Health Per Diem

We are seeking a skilled and compassionate Licensed Practical Nurse to join our team at CenterWell Home Health. As a Home Health LPN, you will play a vital role in providing high-quality patient care and support in the comfort of their own homes.
Key Responsibilities- Provide professional nursing services under the direction of a RN, adhering to state and federal regulations, and CenterWell Home Health policies and procedures.
- Develop and implement individualized care plans, focusing on patient-centered care and outcomes.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
- Conduct thorough assessments, identify patient needs, and develop effective interventions to address those needs.
- Document patient progress, outcomes, and any changes in their condition, ensuring accurate and timely reporting.
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Valid drivers license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
