Licensed Practical Nurse Home Health Per Diem

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Home Health team at CenterWell. As a Home Health LPN, you will play a vital role in providing high-quality patient care and support to individuals in their homes.
Key Responsibilities- Provide Professional Nursing Services: Under the direction of a Registered Nurse (RN), you will provide professional nursing services in compliance with state and federal regulations, as well as CenterWell's policies and procedures.
- Develop and Implement Care Plans: You will work with patients and their families to develop and implement individualized care plans that address their unique needs and goals.
- Conduct Home Visits: You will conduct regular home visits to assess patients' conditions, provide medical care, and educate patients and their families on self-care techniques and disease management.
- Collaborate with Healthcare Teams: You will work closely with RNs, therapists, and other healthcare professionals to ensure seamless care coordination and communication.
- Document Patient Information: You will maintain accurate and detailed records of patient interactions, including progress notes, medication lists, and treatment plans.
- Graduate of an Accredited LPN Program: You must have a diploma or certificate from an accredited Licensed Practical Nursing program.
- Current LPN License: You must hold a current and active LPN license in the state where you will be practicing.
- Valid Driver's License: You must have a valid driver's license and reliable transportation.
- CPR Certification: You must hold current CPR certification.
- 2+ Years of LPN Experience: You must have at least 2 years of experience as an LPN in a clinical setting, preferably in home health or hospice care.
