Job Opportunity

CenterWell Home Health is seeking a skilled Licensed Practical Nurse to join our team. As a Home Health LPN, you will provide high-quality patient care in the comfort of their own homes.

Responsibilities
  • Deliver professional nursing services under the direction of a RN, adhering to state and national standards.
  • Develop and implement individualized care plans to address patient needs and goals.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Provide education and support to patients and families to promote health and wellness.
  • Document patient progress and outcomes in a timely and accurate manner.
Requirements
  • Graduate of an accredited LPN program.
  • Current nursing license in the practicing state.
  • Valid drivers license and reliable transportation.
  • Current CPR certification.
  • Two years of experience as an LPN in a clinical setting, preferably in home health or hospice.
