About the Job

CenterWell Home Health is seeking a skilled Licensed Practical Nurse (LPN) to join our team in providing high-quality home health care to patients in need. As an LPN, you will play a vital role in delivering compassionate and personalized care to our patients, helping them to recover and regain their independence.

Key Responsibilities
  • Provide professional nursing services under the direction of a Registered Nurse (RN) in compliance with state and federal regulations, as well as CenterWell Home Health policies and procedures.
  • Develop and implement individualized care plans to meet the unique needs of each patient, focusing on rehabilitative, preventative, and therapeutic interventions.
  • Collaborate with the healthcare team to ensure seamless care coordination and effective communication with patients, families, and healthcare providers.
  • Conduct thorough assessments, identify patient needs, and develop strategies to address those needs in a timely and effective manner.
  • Document patient progress, outcomes, and any changes to the care plan in a clear and concise manner.
  • Participate in ongoing education and training to stay current with best practices, industry developments, and regulatory requirements.
Requirements
  • Graduate of an accredited Licensed Practical Nursing program or accredited School of Vocational Nursing.
  • Current nursing license in the practicing state.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
