About the Role:
As a Home Health LPN with CenterWell Home Health, you will play a vital role in providing high-quality, patient-centered care to individuals in the comfort of their own homes. Our team is dedicated to delivering compassionate and personalized care, and we are seeking a skilled and compassionate LPN to join our team.
Responsibilities:
- Provide skilled nursing interventions to patients in their homes, including wound care, medication management, and education on disease management.
- Collaborate with the healthcare team to develop and implement individualized care plans that meet the unique needs of each patient.
- Conduct thorough assessments of patients' physical and emotional needs, and develop plans to address these needs.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
- Document patient progress and outcomes in a timely and accurate manner.
Requirements:
- Graduate of an accredited Licensed Practical Nursing program.
- Current nursing license in the practicing state.
- Valid driver's license and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
