Job Summary

CenterWell Home Health is seeking a skilled Licensed Practical Nurse (LPN) to join our team on a per diem basis. As a Home Health LPN, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to deliver comprehensive care.

Key Responsibilities
  • Provide professional nursing services under the direction of a Registered Nurse (RN) in compliance with state and federal regulations, as well as CenterWell Home Health policies and procedures.
  • Develop and implement individualized care plans to meet the unique needs of each patient, focusing on rehabilitation, recovery, and prevention.
  • Conduct thorough assessments, identify patient needs, and develop effective care plans in collaboration with the healthcare team.
  • Administer medications, perform treatments, and provide education to patients and their families on disease management, self-care, and wellness.
  • Communicate effectively with patients, families, physicians, and other healthcare professionals to ensure seamless care coordination.
  • Participate in ongoing education and training to stay current with best practices, industry developments, and regulatory requirements.
Requirements
  • Graduate of an accredited Licensed Practical Nursing program or vocational nursing school.
  • Current nursing license in the practicing state.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
