Position Summary:
The Advanced Practice Nurse will play a crucial role within a collaborative care team, overseeing a select group of patients experiencing CKD stages 3b, 4, 5, and end-stage renal disease (ESRD). This position involves direct patient engagement across various environments, including home visits, virtual consultations, and telephonic interactions. The Nurse Practitioner will partner closely with the care team to deliver exceptional, value-driven healthcare, emphasizing both individual patient needs and broader systemic improvements.
Primary Responsibilities:
- Deliver clinical services for an assigned group of patients in conjunction with the care team.
- Conduct thorough medical histories and physical examinations, and request essential laboratory and diagnostic evaluations.
- Instruct patients regarding their medications, manage prescriptions, and monitor for adverse effects.
- Recognize and address medical issues and obstacles to care.
- Engage in care team meetings and contribute to patient care strategies.
- Coordinate care across multiple settings, including hospitals, skilled nursing facilities, and community organizations.
- Perform structured evaluations and create comprehensive care plans.
- Provide after-hours telephonic support as required.
- Accurately document all patient interactions in a timely manner.
- Assist in the formulation and execution of clinical protocols aimed at enhancing patient outcomes.
- Uphold patient confidentiality and comply with all organizational policies.
Qualifications:
- Licensed Advanced Practice Nurse (APN) in the relevant state.
- Must fulfill clinician credentialing requirements, including background verification, drug screening, and vaccination mandates.
- Possess reliable transportation and a verifiable good driving record.
- Self-driven with the capacity to adhere to established operational protocols.
- Strong aptitude for fostering inclusive relationships with patients, caregivers, and colleagues.
- Experience engaging with diverse community resources.
- Exceptional communication abilities, both verbal and written.
- Comfortable conducting home visits to patients.
- Preference for candidates whose personal experiences resonate with our patient demographic.
