  • The Advanced Practice Nurse Specialist will play a crucial role within a healthcare team, overseeing a select group of patients with Chronic Kidney Disease (CKD) stages 3b, 4, 5, and End-Stage Renal Disease (ESRD).
  • This position entails direct engagement with patients across various environments, including home visits, virtual consultations, and telephonic interactions.
  • The Specialist will collaborate closely with the healthcare team to deliver exceptional, value-driven care, emphasizing both individual patient needs and broader system-level strategies.

Core Responsibilities:

  • Deliver clinical services for a designated patient panel in partnership with the healthcare team.
  • Conduct thorough medical histories and physical examinations, as well as order essential laboratory and diagnostic assessments.
  • Provide patient education regarding medications, manage prescriptions, and monitor for potential side effects.
  • Identify and resolve medical issues and obstacles to care.
  • Engage in team meetings and contribute to patient care strategies.
  • Facilitate care transitions across multiple settings, including hospitals, skilled nursing facilities, and community resources.
  • Perform systematic evaluations and create comprehensive care plans.
  • Offer after-hours telephonic support when necessary.
  • Ensure accurate and timely documentation of all patient interactions.
  • Assist in the formulation and execution of clinical protocols aimed at enhancing patient outcomes.
  • Uphold patient confidentiality and comply with all organizational policies.

Required Qualifications:

  • Licensed Advanced Practice Nurse (APN) in the state of Arizona.
  • Must fulfill client clinician credentialing standards, including background checks, drug screenings, and vaccination requirements.
  • Possess reliable transportation and a documented good driving history.
  • Self-driven with the capability to adhere to established operational guidelines.
  • Strong aptitude for fostering inclusive relationships with patients, caregivers, and healthcare personnel.
  • Experience engaging with diverse community resources.
  • Exceptional communication skills, both verbal and written.
  • Comfortable conducting home visits for patient care.
  • Preference for candidates whose personal experiences resonate with our patient demographic.
