Position Overview:
We are seeking a proficient and experienced individual to fulfill the role of a Nurse Practitioner. This role entails ensuring adherence to healthcare service standards of the Contracted Healthcare Provider (CHP), who is tasked with delivering thorough healthcare evaluations for inmates, examining medical histories, conducting physical assessments, performing diagnostic tests, and diagnosing and managing both prevalent acute and chronic health issues in accordance with established clinical guidelines and protocols.
This role will be pivotal in fostering the development of the Patient Care Center Model (PCCM) and supervising patient care activities in alignment with the model's expectations. This includes (1) collaborating with the CHP to create and execute tailored care plans for inmates with intricate medical requirements and (2) offering education and guidance to CHP personnel on health promotion, disease prevention, and self-management techniques for chronic conditions.
Additionally, this position will evaluate the quality of healthcare services rendered to the inmate population. Develop metrics for outcomes, processes, and structures that can support the PCCM. Ensure that healthcare service delivery achieves desirable outcomes and that any deficiencies are addressed promptly. Analyze and report significant trends and patterns in quality and utilization. Assist in preparations for internal and external audits within the division. Monitor compliance with contractual obligations.

Your core responsibilities will encompass:
- Engaging in multidisciplinary treatment team meetings to discuss inmate cases, share insights, and coordinate care across various disciplines.
- Providing education and training to CHP staff on health promotion, disease prevention, and self-management strategies for chronic conditions.
- Participating in quality improvement initiatives and research projects aimed at enhancing healthcare service delivery within correctional facilities.
- Conducting comprehensive medical chart reviews and offering feedback on the quality of care in managing and treating patients.
- Attending relevant meetings and committees.
- Reviewing patient records to ensure proper documentation of clinical assessments, treatment plans, progress notes, and other pertinent records in accordance with departmental policies and procedures; and evaluating the appropriateness of care.
- Collaborating with contractors, external healthcare providers, and community resources to ensure continuity of care for inmates transitioning from correctional facilities to community settings.
- Drafting policies, protocols, processes, reports, and benchmarks to enhance quality assurance and timely access to care.
- As directed by the ADCRR Medical, Dental, and Mental Health Directors, providing recommendations for updating existing clinical practice guidelines, policies, and procedures.
Knowledge, Skills & Abilities:
Knowledge of:
- ADCRR policies and procedures, Arizona Revised Statutes.
- Advanced nursing principles, theories, and practices.
- Comprehensive understanding of pathophysiology, pharmacology, and medical therapeutics.
- Familiarity with diagnostic procedures, laboratory tests, and result interpretation. Understanding of healthcare delivery systems, including primary, acute, and long-term care.
- Knowledge of healthcare regulations, standards, and best practices related to nursing and advanced practice nursing.

- Clinical Assessment: performing comprehensive health assessments, including physical examinations, medical history reviews, and diagnostic testing.
- Proficiency in diagnosing and managing acute and chronic health conditions, prescribing medications, and ordering diagnostic tests as necessary. Implementing evidence-based therapeutic interventions to promote health, prevent illness, and manage disease.

Selective Preferences:
- Master's degree in Nursing (MSN) from an accredited institution.
- Valid and unrestricted Nurse Practitioner (NP) license in the State of Arizona.
- Minimum of 5 years of experience as a Nurse Practitioner, preferably in a primary care or correctional healthcare environment.
Pre-Employment Requirements:
- Valid and unrestricted Nurse Practitioner license for the State of Arizona.
- Employment is contingent upon passing a background investigation.
- If driving is an essential function of the job, the following requirements apply: any employee operating a motor vehicle on State business must possess and retain a current, valid class-appropriate driver's license, complete all required training, and successfully pass all necessary driver's license record checks. The license must be current, unexpired, and neither revoked nor suspended. Employees who drive on state business are subject to driver's license record checks, must maintain acceptable driving records, and must complete all required driver training. An employee who operates a personally owned vehicle on state business shall maintain the statutorily required liability insurance.
To maintain a safe environment, all employees are required to take a Tuberculosis (TB) test during their New Employee Orientation and annually thereafter.
