Key Responsibilities:- Deliver comprehensive primary care, including preventive services, diagnosis, and management of health conditions.
- Maintain accurate documentation of all patient interactions in compliance with healthcare standards.
- Collaborate as an active member of a patient-centered healthcare team.
- Contribute to organizational goals and initiatives as required.
- Graduation from an accredited Nurse Practitioner Program with certification from the American Nurses Credentialing Center or the American Academy of Nurse Practitioners.
- Possession of a permanent and unrestricted Arizona nurse practitioner license.
- Current board certification in family medicine is required.
- Active medication dispensing license (DEA) is necessary.
- A minimum of three years’ experience in a family practice or similar environment is essential; experience in community health settings is preferred.
- Current ACLS certification is required.
- Valid Arizona driver's license is mandatory, with no recent DUI/DWI or reckless driving offenses.
- Successful completion of a criminal background check and acquisition of a Class I Fingerprint Clearance Card within the first ninety days of employment.
- Passing substance abuse testing upon hiring and during employment is required.
- Updated immunization records must be provided at the time of employment.
- Current Tuberculosis (TB) skin test results are required upon hiring.
- First Aid/CPR certification must be obtained within ninety days of employment.
- Strong understanding of evidence-based family medicine practices.
- Proficient in delivering diagnostic, preventive, and therapeutic patient care.
- Knowledgeable in public health and community health principles.
- Ability to work collaboratively with diverse staff members.
- Effective communication skills, both written and verbal.
- Proficient in using electronic health records and Microsoft Office applications.
- Ability to build rapport and trust with patients and their families.
- Commitment to maintaining confidentiality and ethical standards in patient care.
