Role Summary

The Site Coordinator is responsible for leading or co-leading a dedicated team at a designated school site, ensuring the effective application of the CIS Model. This role serves as a critical link between school leadership and the broader CIS of Nevada team. The coordinator will develop, plan, and oversee the execution of the annual School Support Plan, which encompasses various tiers of service aimed at addressing student needs.

Key Responsibilities

Campus Assessment and Planning

  • Conduct thorough assessments of campus needs in collaboration with school administration, educators, families, and students to identify and coordinate necessary community resources.
  • Address specific campus challenges by mobilizing appropriate resources.
  • Organize and implement special events and guest speaker sessions that align with the CIS School Support Plan.
  • Facilitate parental engagement initiatives as part of the School Support Plan.
  • Assist in fostering partnerships with community agencies to enhance resource availability.
  • Work alongside school personnel to ensure compliance with district policies while addressing attendance, behavioral, and academic challenges.

Case Management

  • Recruit students for integrated support services with appropriate parental consent.
  • Develop and maintain individualized Student Support Plans for case-managed students.
  • Monitor and report on student progress regarding attendance, behavior, and academic performance.
  • Ensure all documentation adheres to program guidelines and is submitted in a timely manner.
  • Facilitate access to community health and human services for students and their families.

Documentation and Reporting

  • Maintain accurate case management records in the required format and within established deadlines.
  • Prepare monthly reports and correspondence as needed.
  • Communicate effectively with CIS of Nevada leadership regarding any issues impacting school relationships.

Data Management

  • Ensure timely enrollment of new students into the data management system.
  • Document service data promptly to meet program requirements.
  • Report any technical issues with the data management system to the appropriate personnel.

School Relations

  • Promote collaboration between school staff and the CIS team to enhance service integration.
  • Provide constructive feedback to school and agency personnel.
  • Maintain open lines of communication with school administration and staff.

Professional Development

  • Participate in all required meetings and training sessions.
  • Engage in ongoing professional development opportunities.

Qualifications

  • Experience working with underserved youth and in case management is preferred.
  • Strong interpersonal, written, and verbal communication skills, along with proficiency in data management.
  • A Bachelor's degree or equivalent experience in a relevant field is required.
  • Must successfully pass background checks and drug screenings.
  • Bilingual skills in Spanish and English are advantageous.

Work Environment

  • Work primarily indoors in a controlled environment, with occasional outdoor exposure.
  • Moderate noise levels typical of a classroom setting.
  • Use of personal transportation may be required.
