With a robust network of community partners, we effectively coordinate resources and services, integrating community support within educational settings to assist students in overcoming barriers they face.

Role Summary

The individual in this role will lead or co-lead a dedicated team at a designated school site, implementing the CIS Model effectively. This position serves as a crucial link between the school leadership and the CIS of Nevada leadership, ensuring the successful execution of the annual School Support Plan, which encompasses Tier I, Tier II, and Tier III services. Collaboration with school personnel, families, community organizations, and students is essential to guarantee the seamless delivery of services that benefit students and their families.

Key Responsibilities

Campus Development

  • Evaluate the needs of the campus in collaboration with school administration, educators, families, and students, and coordinate community resources to address these needs.
  • Identify and address specific campus or student challenges by mobilizing appropriate resources.
  • Organize special events and guest speaker sessions for students as part of the approved CIS School Support Plan.
  • Facilitate parental engagement activities aligned with the CIS School Support Plan.
  • Assist in fostering partnerships with community agencies to enhance resource availability.
  • Work alongside school staff to provide or arrange necessary services addressing attendance, behavior, academic performance, and other student needs, ensuring adherence to district policies.

Case Management

  • Recruit students for Tier II and Tier III support services with parental consent.
  • Develop and oversee individual Student Support Plans for all case-managed students.
  • Monitor attendance, behavior, and academic performance goals for each student.
  • Ensure all documentation is completed in accordance with program guidelines and timelines.
  • Maintain accurate records of services provided as per program requirements.
  • Coordinate with community health and human service agencies to facilitate necessary services for students and their families.

Documentation & Reporting

  • Complete case management documentation in the prescribed format within established deadlines.
  • Prepare monthly reports in the required format, adhering to timelines.
  • Maintain effective communication through necessary correspondence.
  • Report any issues affecting school relationships or emergencies to leadership.

Data Management

  • Ensure the enrollment of new students into the data management system as required.
  • Input service data for all students within the specified timeframe.
  • Report any issues with the data management system promptly.
  • Submit progress data quarterly and at the end of the academic year as required.

School Relations

  • Encourage collaboration between school staff and the CIS team.
  • Work with school personnel to integrate CIS services and stay informed about school and community activities.
  • Provide constructive feedback to school and agency staff.
  • Foster a positive image of CIS within the school community.
  • Maintain regular communication with school administration and staff.

Meetings, Training, and Professional Development

  • Attend all relevant meetings and training sessions.
  • Engage in personal and professional development opportunities.
  • Complete required certification within the designated timeframe.

Additional Responsibilities

  • Perform other duties as assigned.

Qualifications

  • Experience working with youth from low-income backgrounds and in case management is preferred.
  • Strong interpersonal, written, and verbal communication skills, along with proficiency in computer and database management.
  • A Bachelor's degree or equivalent experience in health and human services or a related field.
  • Ability to pass background checks and drug screenings is required.
  • Bilingual skills in Spanish and English are desirable.

Work Environment

  • Work is primarily indoors in a controlled climate, with occasional exposure to outdoor conditions.
  • Noise levels typical of a classroom setting.
  • Use of personal vehicle may be required.
