We are seeking a highly motivated and compassionate Outreach Specialist Level 1 to join our San Francisco Homeless Outreach Team (SFHOT). As a key member of our team, you will play a vital role in engaging and stabilizing individuals experiencing homelessness, connecting them with essential services, and advocating for their needs.
Key Responsibilities- Conduct outreach in assigned neighborhoods to engage individuals experiencing homelessness and assess their needs.
- Triage calls from the community and dispatch teams and responders accordingly.
- Engage individuals, assess their needs, and provide support and referrals for essential services.
- Collaborate with service providers to facilitate access to care and advocate for clients' needs.
- Work collaboratively with the DPH Street Medicine Team to link individuals to transitional primary care.
- Direct experience working with individuals experiencing homelessness or complex medical and behavioral health concerns.
- Valid California driver's license and a clean driving record.
- Ability to work well with diverse staff and clientele, including cultural, language, and other diversity considerations.
- Basic knowledge of the client population and their complex needs.
- Ability to speak, read, and write clearly in English.
- Previous experience or training in street outreach and case management.
- Knowledge of San Francisco neighborhoods and community resources.
- Knowledge of the San Francisco Health Provider network.
This position requires frequent travel to various locations, including outdoor settings and occasional indoor office work. The ideal candidate will be able to work in a fast-paced environment, prioritize tasks, and maintain a high level of professionalism and compassion.
