Job Summary

We are seeking a highly skilled Clinical Case Manager to join our team at Hope Network Behavioral Health. As a Clinical Case Manager, you will be responsible for providing comprehensive psychosocial assessments, individual and group therapy, and case management services to our clients.

Key Responsibilities
  • Perform comprehensive psychosocial assessments to identify client needs and develop individualized treatment plans.
  • Provide individual and group therapy sessions to clients, utilizing evidence-based practices and techniques.
  • Administer and score standardized psychological assessment measures to evaluate client progress and adjust treatment plans as needed.
  • Coordinate and ensure client involvement in the Person-Centered Planning Process (PCP) to promote client-centered care.
  • Meet with clients face-to-face as deemed medically necessary to provide support and guidance.
  • Develop and implement individualized plans of service for each client, taking into account their unique needs and goals.
  • Participate in legal proceedings, petitioning, and testifying in clinical emergencies as needed.
  • Coordinate care with internal and external stakeholders, including CMH, guardians, advocates, and other healthcare providers.
  • Facilitate the coordination of client intakes, discharges, and transfers to ensure seamless care transitions.
  • Coordinate with nursing staff to provide behavioral supports before and after appointments, and accompany clients to appointments as needed.
Requirements
  • Master's degree in Social Work, Psychology, or Counseling required.
  • Current licensure as a Licensed Master Social Worker (LMSW), Licensed Psychologist (LP), Limited Licensed Psychologist (LLP), Temporary Limited Licensed Psychologist (TLLP), Limited License Counselor (LLC), or Licensed Professional Counselor (LPC) required.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
  • Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare providers.
  • Strong problem-solving and critical thinking skills, with the ability to analyze complex situations and develop effective solutions.
