Position Overview

Role Summary
As a dedicated Case Manager, you will deliver exceptional, women-centered case management services aimed at promoting recovery and wellness for individuals facing substance use and mental health challenges. Your focus will be on supporting pregnant and/or parenting individuals. You will collaborate with an interdisciplinary team to align with clinical treatment objectives, while actively engaging with the community and establishing essential referrals.

Core Responsibilities
Success in this role is achieved through the cultivation of meaningful and respectful relationships, grounded in trauma-informed care, hope, and evidence-based practices. A commitment to understanding and addressing bias, equity issues, and the complexities of individual experiences is essential for growth within our organization.

Services will be delivered both in community settings and through in-person office interactions. Qualifications
To be considered for this position, candidates should possess the following:

  • A Bachelor's degree in Social Work
  • Experience in the Substance Use Disorder (SUD) field
  • Demonstrated experience as a Case Manager
  • A valid driver's license with a clean driving record
