Position Overview

Role Summary
As a dedicated Case Manager, you will deliver high-quality, women-centered case management services aimed at promoting recovery and wellness for individuals facing substance use and mental health challenges. Your focus will be on supporting pregnant and/or parenting individuals, collaborating with an interdisciplinary team to meet clinical treatment objectives.

Key Responsibilities
Engage with community partners to foster connections and facilitate referrals. Achieve positive outcomes through respectful and intentional relationships grounded in trauma-informed care, hope, and evidence-based practices. A commitment to understanding and addressing bias and equity issues, as well as navigating successes and challenges with empathy, is essential for success in this role.

Work Environment
Services will be provided both in community settings and through in-person office interactions. Qualifications
To be considered for this position, candidates should possess the following:

  • A Bachelor's degree in Social Work
  • Experience in the Substance Use Disorder (SUD) field
  • Demonstrated experience as a Case Manager
  • A valid driver's license and a clean driving record
