About the Role

We are seeking a highly skilled Patient Navigation Coordinator to join our team at UW Health. As a key member of our Breast Center, you will play a vital role in ensuring that patients receive seamless and compassionate care.

Key Responsibilities
  • Assess patients' barriers to care and make appropriate referrals to remove obstacles to effective breast health services.
  • Advocate for and support patients and caregivers, collaborating with our team and support services to provide equitable care and outcomes.
  • Recruit and match patients for our Breast Cancer Peer Support Advocacy Program.
  • Promote the Breast Center program through community outreach activities with local groups, organizations, and institutions that are stakeholders in cancer health disparities.

Requirements
  • Minimum - Bachelor's Degree in social work, counseling, nursing, health education, or a related field.
  • Preferred - Master's Degree in social work, counseling, nursing, health education, or a related field.
  • Minimum - One (1) year of experience working with underserved patient populations.
  • Preferred - RN, LPN, or Social Worker with oncology experience.
