Job Overview

Luminary Hospice is seeking a compassionate and skilled Certified Hospice Aide to join our team in Carmel, Indiana. As a key member of our interdisciplinary group, you will provide high-quality care and support to patients and their families, promoting comfort and dignity in their final stages of life.


Responsibilities
  1. Provide personal care services to patients, including bathing, grooming, and dressing, as assigned.
  2. Assist patients with daily living activities, such as feeding, toileting, and ambulation, as needed.
  3. Monitor patients' conditions and report any changes or concerns to the Case Manager.
  4. Document patient care and observations accurately and thoroughly.
  5. Collaborate with the interdisciplinary team to develop and implement patient care plans.
  6. Provide emotional support and companionship to patients and their families.
  7. Assist with light housekeeping and laundry tasks, as assigned.
  8. Participate in quality assessment and performance improvement activities.

Requirements

To be successful in this role, you will need:

  • A high school diploma or equivalent.
  • Current certification as a Hospice Aide in the state of Indiana.
  • At least 1-3 years of experience in hospice care or a related field.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Physical stamina to perform tasks that require standing, walking, and lifting.

  • Communication: Ability to effectively communicate with patients, families, and healthcare team members.
  • Customer Service: Demonstrated ability to provide excellent customer service and meet the needs of patients and families.
  • Emotional Intelligence: Ability to manage one's emotions and interact successfully with others.
  • Time Management: Ability to manage time effectively and prioritize tasks.

Work Environment

The work environment for this role is typically quiet and requires minimal physical demands. However, the ability to lift and move up to 50 pounds may be required in some situations.


Job Limitations

This role does not involve the administration of medications, taking physician's orders, or performing procedures that require the training, knowledge, and skill of a licensed nurse.
