Job Title: Hospice Registered Nurse Case Manager

Job Summary:

The Hospice Registered Nurse Case Manager is responsible for planning, organizing, and directing hospice care for patients and their families. This role requires a strong understanding of community health education and experience in nursing, with a focus on end-of-life care.

Key Responsibilities:

  1. Conduct comprehensive assessments of patients and their families to determine hospice needs.
  2. Develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients and their families.
  3. Collaborate with the interdisciplinary team to provide high-quality, patient-centered care.
  4. Ensure compliance with regulatory requirements and maintain accurate records.
  5. Provide education and support to patients and their families on end-of-life care and hospice services.
  6. Participate in quality improvement initiatives and contribute to the development of policies and procedures.

Requirements:

  • Bachelor's or Associate's degree in nursing or a related field.
  • Active nursing license in good standing.
  • At least 2-5 years of experience in hospice care or a related field.
  • Knowledge of Medicare Hospice Certification and CHAP guidelines.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.

The Hospice Registered Nurse Case Manager will work in a fast-paced, dynamic environment with a focus on providing high-quality patient care. The ideal candidate will be able to work effectively in a team setting and prioritize tasks to meet deadlines.

Physical Demands:

The Hospice Registered Nurse Case Manager will be required to sit for extended periods, use a computer, and communicate with patients and staff. The ideal candidate will be able to lift up to 50 pounds and move around the office or patient care areas as needed.
