POSITION SUMMARY:

The hospice aide serves as a vital member of the interdisciplinary team, operating under the guidance of a registered nurse to deliver essential services that cater to the personal needs of patients and enhance their comfort. This role involves careful observation of the patient, timely reporting of findings, and thorough documentation of care provided. The hospice aide is assigned in a manner that ensures the quality, continuity, and safety of patient care.

KEY RESPONSIBILITIES:
  1. Addressing personal care requirements based on patient assignments and responding to requests promptly. In case of inability to perform a task, immediate reporting to the Case Manager is essential.
  2. Utilizing effective communication techniques with patients and families, ensuring ethical and respectful care, while maintaining confidentiality.
  3. Ensuring patient safety and proper usage of equipment (e.g., foot stools, side rails, oxygen).
  4. Administering personal care services such as bathing, back rubs, oral hygiene, and changing bed linens as assigned.
  5. Assisting patients with dressing and undressing as required.
  6. Supporting patients during mealtime as directed.
  7. Measuring and documenting vital signs including oral, rectal, and auxiliary temperatures, pulse, respiration, and blood pressure as ordered, demonstrating appropriate skills competency.
  8. Monitoring and caring for the patient's skin to prevent tissue breakdown over bony prominences.
  9. Reporting significant changes in the patient's condition to the Case Manager, while being mindful of the caregiver's dynamics and interpersonal issues.
  10. Facilitating ambulation and exercise as instructed by the hospice nurse or therapist.
  11. Providing normal range of motion exercises and assisting with transfers/positioning as directed.
  12. Offering assistance with self-administered medications as permitted by state regulations, such as reminders.
  13. Providing assistance with bedpans and urinals, along with light laundry tasks as assigned.
  14. Carrying out range of motion and other simple procedures as an extension of therapy services as ordered, demonstrating appropriate skills competency.
  15. Offering respite for patients, families, and caregivers when present on-site, as appropriate.
  16. Maintaining cleanliness and orderliness in the patient's living area as assigned.
  17. Adhering to the organization's documentation and care standards, along with professional conduct expectations.
  18. Participating in quality assessment and performance improvement initiatives.
  19. Executing additional responsibilities as assigned.
QUALIFICATIONS:

Possession of an active hospice aide certification as required by state regulations. A minimum of 1-3 years of experience in hospice or palliative care, or training in end-of-life care is preferred. Knowledge of hospice care principles and services provided to patients and families through an interdisciplinary approach is essential. Proficiency in MS Office applications is preferred, along with the ability to learn job-specific software systems. Strong organizational, verbal, and written communication skills are necessary, along with analytical skills for effective problem-solving. Attention to detail and accuracy are critical.

While fulfilling job duties, the employee frequently sits and uses hands for tasks. Occasionally, the employee stands and walks. Lifting and/or moving up to 50 pounds may be required on occasion.

LIMITATIONS:

The hospice aide is not permitted to engage in activities considered nursing practices as defined by the Nurse Practice Act. Specifically, the hospice aide will not administer medications, take physician orders, or perform procedures that necessitate the training, knowledge, and skills of a licensed nurse, such as sterile techniques.
