Job Title: Home Care and Hospice Liaison

At Liberty Home Care and Hospice, we are seeking an experienced Home Care and Hospice Liaison to join our team. As a Home Care and Hospice Liaison, you will play a critical role in coordinating the transfer of patients to home health and hospice services, ensuring uninterrupted continued medical care.

Key Responsibilities:

  • Coordinate health care institution and physician office activities to facilitate patient transfers.
  • Establish relationships with health care institutions to manage patient transfers and provide education on home health and hospice services.
  • Coordinate patient services activities, including scheduling and follow-up.
  • Provide education to health care institutions and physicians on home health and hospice services.
  • Attend community events to promote Liberty Home Care and Hospice services.

Requirements:

  • Bachelor's degree or LPN, RN, SP, LPT, MSW.
  • Two years' experience working with home health, hospice, hospitals, physicians, HMO's, outpatient services or social services concerning public relations or office procedures.
  • Knowledge of Medicare/Medicaid home health and hospice regulations preferred.
  • Ability to coordinate patient transfers and provide education on home health and hospice services.
  • Strong communication and interpersonal skills.
