Job Summary

As a Registered Nurse with Duke HomeCare & Hospice, you will have the opportunity to provide compassionate care to patients in their homes or in our inpatient hospice facility. Our team is dedicated to delivering exceptional patient care and working collaboratively with patients, families, and healthcare providers to achieve the best possible outcomes.

Responsibilities
  • Plan, provide, and document professional nursing care for patients in accordance with the NC nurse practice act, physician orders, and established policies and procedures.
  • Assess patient needs, develop individualized care plans, and implement interventions to achieve patient goals.
  • Collaborate with healthcare teams to evaluate and improve patient care, including participation in weekly interdisciplinary team meetings and case conferences.
  • Document care and services provided in accordance with DHCH Policy and Procedures and regulatory standards.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
Requirements
  • Current North Carolina RN licensure or compact RN licensure in participating state.
  • Current State of North Carolina Cardio-Pulmonary Resuscitation Certificate (CPR) (American Heart Association only).
  • Two years of med-surgical, critical care, or home health/hospice/infusion experience.
  • Valid driver's license and current automobile insurance coverage.
Preferred Qualifications
  • Prior experience in Home Health, Hospice, or Infusion.
  • Prior OASIS and PDGM experience preferred for home health.
