Key Responsibilities:- Coordinate healthcare institution and physician office activities to ensure seamless patient transitions.
- Establish and maintain relationships with healthcare institutions to facilitate the transfer of patients to home care and hospice services.
- Coordinate patient services activities, including health education and community outreach.
- Assist with continuous recruitment activities to attract qualified staff.
- Bachelor's degree or LPN, RN, SP, LPT, MSW.
- Two years' experience working with home health, hospice, hospitals, physicians, HMO's, outpatient services, or social services.
- Knowledge of Medicare/Medicaid home health and hospice regulations.
- Strong communication and interpersonal skills.
- Ability to travel as necessary.
