Key Responsibilities:
  • Coordinate healthcare institution and physician office activities to ensure seamless patient transitions.
  • Establish and maintain relationships with healthcare institutions to facilitate the transfer of patients to home care and hospice services.
  • Coordinate patient services activities, including health education and community outreach.
  • Assist with continuous recruitment activities to attract qualified staff.
Requirements:
  • Bachelor's degree or LPN, RN, SP, LPT, MSW.
  • Two years' experience working with home health, hospice, hospitals, physicians, HMO's, outpatient services, or social services.
  • Knowledge of Medicare/Medicaid home health and hospice regulations.
  • Strong communication and interpersonal skills.
  • Ability to travel as necessary.
