Job Summary

We are seeking an experienced Home Care and Hospice Liaison to join our team at Liberty Health. As a key member of our healthcare team, you will play a vital role in coordinating patient services and ensuring seamless transitions between healthcare settings.

Key Responsibilities:

  • Coordinate healthcare institution and physician office activities to facilitate patient transfers to home care and hospice services.
  • Establish and maintain relationships with healthcare institutions to promote efficient patient care.
  • Develop and implement patient-centered care plans in collaboration with healthcare teams.
  • Provide education and support to patients, families, and healthcare providers on home care and hospice services.
  • Collaborate with branch management and other stakeholders to ensure effective communication and coordination.
  • Assist with recruitment and retention efforts to attract qualified healthcare professionals.

Requirements:

  • Bachelor's degree or equivalent in a healthcare-related field.
  • Minimum two years of experience in home health, hospice, hospitals, physicians, HMOs, outpatient services, or social services.
  • Knowledge of Medicare/Medicaid home health and hospice regulations.
  • Strong communication, interpersonal, and problem-solving skills.
  • Ability to travel as necessary to meet with healthcare providers and patients.
