Job Title: Hospice Community Liaison

At Liberty Home Care and Hospice, we understand the importance of providing patients with the best possible care in the comfort of their own homes. As a Hospice Community Liaison, you will play a vital role in ensuring that our patients receive seamless and uninterrupted medical care.

Key Responsibilities:
  • Coordinate with healthcare institutions and physicians to ensure smooth patient transfers to home health and hospice services.
  • Develop and implement community education programs to promote our services and build relationships with local healthcare providers.
  • Assist in the recruitment and retention of qualified staff, ensuring that our patients receive the highest level of care.
  • Collaborate with our healthcare team to ensure compliance with Medicare/Medicaid regulations and guidelines.
  • Provide exceptional customer service and communication skills to patients, families, and healthcare providers.

Requirements:

  • Bachelor's degree or equivalent in a related field.
  • 2+ years of experience in home health, hospice, or a related field.
  • Strong knowledge of home health and hospice services, including federal, state, and ACHC guidelines.
  • Excellent interpersonal and communication skills.
  • Ability to travel as necessary.
