About the Role

We are seeking a skilled Registered Nurse to join our Hospice Home and Partners team at Duke Health. As a nurse in this role, you will have the opportunity to provide compassionate care to patients and their families in the comfort of their own homes.

Key Responsibilities
  • Provide professional nursing care to patients in accordance with the NC nurse practice act, physician orders, and established policies and procedures.
  • Assess patient needs and develop individualized plans of care to ensure comprehensive and effective treatment.
  • Collaborate with healthcare teams to evaluate and improve patient care, including participation in weekly interdisciplinary team meetings and case conferences.
  • Document patient care and services provided in accordance with Duke Health policies and regulatory standards.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
Requirements
  • Current North Carolina RN licensure or compact RN licensure in participating state.
  • Current State of North Carolina Cardio-Pulmonary Resuscitation Certificate (CPR) (American Heart Association only).
  • Two years of med-surgical, critical care, or home health/hospice/infusion experience.
  • Valid driver's license and current automobile insurance coverage.
Preferred Qualifications
  • Prior experience in Home Health, Hospice, or Infusion.
  • Prior OASIS and PDGM experience preferred for home health.
