Job Summary:

Liberty Health is seeking an experienced Home Care and Hospice Liaison Specialist to join our team. As a key member of our healthcare team, you will be responsible for coordinating healthcare institution and physician office activities, establishing relationships with healthcare institutions, and facilitating the transfer of patients to home care services.

Key Responsibilities:

  • Coordinate healthcare institution and physician office activities to ensure seamless patient care transitions.
  • Establish and maintain relationships with healthcare institutions to facilitate the transfer of patients to home care services.
  • Coordinate patient services activities, including health education and community outreach.
  • Develop and implement strategies to improve patient outcomes and satisfaction.
  • Collaborate with healthcare professionals to ensure continuity of care.
  • Assist with recruitment and retention of qualified healthcare staff.
  • Stay up-to-date with federal, state, and ACHC guidelines related to home health and hospice services.

Requirements:

  • Bachelor's degree or equivalent in a related field.
  • Minimum 2 years of experience in healthcare, preferably in home health, hospice, or hospital settings.
  • Strong knowledge of Medicare/Medicaid home health and hospice regulations.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and analytical skills.
