Position Summary
This role is pivotal in ensuring that clients exit treatment with a comprehensive aftercare strategy, secure housing, and access to further necessary services. The Aftercare Coordinator plays a crucial role in achieving organizational goals related to client census and budget management.

Key Responsibilities

• Deliver high-quality direct and indirect care to individuals facing substance use challenges.

• Evaluate clients' ongoing care and referral requirements.

• Build strong relationships with clients to foster trust and support.

• Gather essential information and consent from clients, families, and relevant referral sources.

• Facilitate referrals to mental health and substance use treatment facilities for continued care.

• Coordinate interviews and placements at halfway houses.

• Collaborate closely with counselors and the multidisciplinary team.

• Attend and contribute to program, clinical, and administrative meetings.

• Actively promote the facility's referral initiatives.

• Engage in mandatory training and orientation sessions.

• Perform additional duties as required.

Supervisory Role
This position does not include supervisory responsibilities.

Qualifications
Education and Experience
• A high school diploma is required; a Bachelor's degree in a human services field is preferred.

Knowledge, Skills, and Abilities
To excel in this role, candidates must demonstrate the following:

• Strong oral and written communication skills, along with effective interpersonal abilities.

• Familiarity with clinical care levels, primary funding sources, and referral networks.

• Proficient problem-solving skills, with the ability to analyze and synthesize information.

• Capability to manage crisis situations with appropriate responses.

• Understanding of federal and state regulations and standards applicable to the field.

Physical Requirements
While performing the duties of this position, the employee is frequently required to communicate verbally and audibly. Regular tasks include sitting, using hands for manipulation, and reaching with arms. Occasional physical activities may involve climbing stairs, standing, walking, bending, or lifting up to 25 pounds. Specific vision capabilities required include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.

Work Environment
The work environment is typically moderately quiet and indoors. Scheduling will depend on operational needs, and personal transportation is necessary. Employees may encounter exposure to health risks from clients and may face challenging situations.
