Key Responsibilities:
Under the general supervision of the unit Supervisor, this individual will provide service coordination for eligible participants who qualify for Medicaid under Community Health Choices.
Service Coordinators are tasked with delivering ongoing service coordination services to eligible participants, which includes:
- Conducting comprehensive assessments to determine needs.
- Linking participants to necessary services and resources.
- Communicating vital information to participants and their support teams.
- Developing and maintaining service plans that reflect participant strengths and needs.
Essential Duties:
- Assess needs and complete thorough evaluations.
- Engage in continuous assessment of participants, ensuring they are connected to required services.
- Monitor adherence to service plans and participant satisfaction.
- Maintain accurate documentation of services provided.
- Provide ongoing support and crisis intervention as needed.

Qualifications:
(1) Registered Nurse (RN); or (2) a bachelor's degree in Social Work, Psychology, or related fields with practicum experience; or (3) a minimum of three (3) years of experience in a social service or healthcare-related setting.
Employment Prerequisites:
Valid automobile insurance, Pennsylvania Driver's License, acceptable Motor Vehicle Record Check, Act 33 and Act 34 Clearances, FBI Fingerprint Clearance, Social Security Card, Immigration and Naturalization (I-9), Drug Screening Clearance. Service Coordinators providing services within the Nursing Home project must complete TB testing.
Working Environment:
Comfortable working in an open office setting and utilizing virtual communication technology. Travel throughout the coverage area is required and may occur in various conditions. Must be able to lift at least 15 pounds.
