Key Responsibilities:

Under the general supervision of the unit Supervisor, this individual will provide service coordination for eligible participants who qualify for Medicaid under Community Health Choices.

Service Coordinators are tasked with delivering ongoing service coordination to eligible participants, which includes continuous monitoring to ensure individual health, safety, and wellness; connecting to community resources; sharing information with participants; and developing and maintaining service plans that outline participant strengths, needs, and services.

Core Duties:
  • Conduct needs assessments and complete comprehensive evaluations.
  • Engage in ongoing assessments of participants, including thorough evaluations and linkage to services.
  • Ensure participants are referred to necessary and/or approved services.
  • Develop and maintain plans that reflect participant strengths and needs.
  • Provide ongoing monitoring to ensure adherence to plans and satisfaction with services, while assessing the ongoing safety and well-being of participants.
  • Communicate priorities and information to participants and their designated team members.
  • Monitor and ensure that assigned participants maintain Medicaid eligibility.
  • Maintain accurate and up-to-date records, including writing service notes for all services provided.
  • Foster positive working relationships with participants and identified team members to ensure ongoing support is provided.

Qualifications:
(1) Registered Nurse (RN); or (2) a bachelor's degree in Social Work, Psychology, or related fields with practicum experience; or (3) at least three (3) years of experience in a social service or healthcare-related setting.

Employment Requirements:
Automobile and Valid Insurance, Valid Pennsylvania Driver's License, acceptable Motor Vehicle Record Check, Act 33 and Act 34 Clearances, FBI Fingerprint Clearance, Social Security Card, Immigration and Naturalization (I-9), Drug Screening Clearance. Service Coordinators providing services within the Nursing Home project must complete TB testing.

Working Environment:
Comfortable working in an open office setting and communicating through virtual technology. Travel throughout the coverage area is required and may occur in adverse conditions. Must be able to lift at least 15 pounds.
