Job Summary

We are seeking a highly skilled and experienced Nurse Placement Coordinator to join our team at Children's Healthcare of Atlanta. As a key member of our interdisciplinary healthcare team, you will be responsible for assessing, determining, and coordinating the placement of patients in accordance with clinical standards and guidelines.

Key Responsibilities
  • Perform initial discharge planning evaluations and emergency department assessments for patients
  • Provide supervision of Licensed Practice Nurse Navigator activities
  • Cultivate and maintain effective interaction/communication with medical staff, nursing staff, social workers, and others to drive the care coordination process
  • Communicate with home health agencies, third-party payors, and other community resources as needed to coordinate patient needs
  • Facilitate clinical practice guidelines at the patient level
  • Perform outpatient and clinic care coordination and monitor patients' care amongst the hospital, clinic, and home
  • Facilitate care via telehealth to assist with patient needs and concerns
  • Provide health promotion activities and education for caregivers and staff at assigned facilities
  • Refer cases identified as risk management issues, peer review issues, or quality issues to appropriate personnel
  • Coordinate patient placement activities to optimize patient flow, patient outcomes, admission appropriateness, and cost efficiencies
Requirements
  • 5 years of experience in nursing with 2 years of experience in critical care or emergency nursing with advanced clinical assessment skills
  • Bachelor of Science in Nursing
  • Experience in progressive leadership and training as house supervisor or bed placement coordinator
  • Experience in psychiatric or behavioral health
  • Experience in a medical inpatient/emergency department setting working with pediatric and adolescent patient populations
  • Experience with behavioral mental health facilities and placement procedures
Preferred Qualifications
  • Certified Case Manager (CCM) or Psychiatric Mental Health (PMH-BC) Certification
  • Basic Life Support (BLS) certification
