Position Overview:
As a Patient Flow Nursing Coordinator, you will be responsible for evaluating, determining, and orchestrating the placement of all patient admissions and transfers, both elective and emergent, in line with established clinical standards and protocols. Your role will be vital in ensuring that patients are placed in the appropriate level of care, utilizing established algorithms and guidelines while upholding our commitment to exceptional customer service.
Qualifications:
- Minimum of 5 years of nursing experience.
- At least 2 years of experience in critical care with advanced clinical assessment capabilities.
- Bachelor of Science in Nursing.
- Experience in progressive leadership roles, such as house supervisor.
- Graduation from an accredited nursing program.
- Bachelor of Science in Nursing is preferred.
- Registered Nurse licensure in Georgia or through the Enhanced Nurse Licensure Compact.
- Basic Life Support (BLS) certification within 30 days of employment.
- Pediatric Advanced Life Support (PALS) certification within 90 days of employment.
- Ability to manage multiple, rapidly changing priorities and make swift decisions.
- Proficient in integrating pathophysiology, psychology, and sociology into pediatric care.
- Strong verbal and written communication skills, with the ability to resolve conflicts and foster collaboration.
- Proficient in various software applications, including Microsoft Office and database management.
- Must achieve a passing score of 80% or higher on the Basic Windows Skill Assessment within 30 days of employment.
- Coordinate patient placement activities through the Transfer Center to enhance patient flow, outcomes, and admission appropriateness while collaborating with house supervisors, physicians, and nursing staff.
- Maintain and manage assigned databases.
- Assist in the collection of data to identify and resolve flow-related issues.
- Analyze and interpret outcome data to identify areas for improvement and support changes in systemwide processes.
- Prepare reports as required.
- Actively participate in organizational initiatives that affect patient flow.
- Support ongoing performance and quality improvement efforts.
- Communicate effectively with leadership to ensure awareness of key decision factors.
- Serve as a clinical resource for patient placement, ensuring efficient Transfer Center operations to minimize wait times.
- Foster positive relationships and demonstrate excellent customer service in all interactions.
- Supervise the Bed Placement Coordinator.
- Encourage and develop team members by creating an environment that promotes professional growth and problem-solving.
- Assist with the hiring process for the Transfer Center.
- Support the orientation and ongoing education of peers.
