We are seeking a skilled and compassionate Hospice Nurse Case Manager to join our team at Deaconess Hospice. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality patient care and support to individuals and their families during their end-of-life journey.
Key Responsibilities- Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
- Develop and implement individualized plans of care, taking into account the patient's physical, psychosocial, and environmental needs.
- Collaborate with patients, families, attending physicians, and other healthcare professionals to ensure seamless care coordination.
- Document patient progress, problems, goals, interventions, and responses to care in a timely and accurate manner.
- Participate in ongoing education and training to stay current with best practices in hospice care.
- Current RN licensure in the state of practice and one year of clinical experience.
- Current CPR Certification.
- Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
